EPHS Athletic Clearance - Soccer

ImPACT Baseline test on your clearance date.  (All freshman, new, or transfer students who have not been ImPACT tested.) Items Needed:  Complete Athletic Code of Conduct (see website), copy of current insurance card, copy of current physical or bring copy of new physical.  Pick up physical form in the Athletic Office.
